Our Palliative Care Program is designed to improve the quality of life for residents with chronic or life-threatening illnesses.
Palliative care is specialized medical care for residents with chronic or life-threatening illness, focused on aligning resident goals to care outcomes. Our specially trained palliative care team communicates with residents and their families to identify health care goals and preferences, manage symptoms such as pain, and make advanced care planning decisions about treatments. Palliative care is based on the needs and desires of the resident, not on the resident’s prognosis. The goal is to improve the quality of life for both the resident and the family.

Each person in our secure memory care unit receives compassionate care and assistance to better enable them to live fully and at the highest functional level possible in a safe, comfortable, supportive environment.
For those who provide home care to a loved one with memory challenges, a respite program is available as well. For more information on this program, please contact Jennifer Lasman, Admissions Director at admissions@hanoverhill.com.
