Essential Duties and Responsibilities:

  • Meet with and solicit advice from other department supervisors concerning physical therapy, assist in identifying and correcting problem areas, and/or the improvement of services.
  • Review requests for physical therapy and physicians/ orders, interview patient to determine type of treatment and schedule therapy.
  • Evaluate type of therapy most desirable after consulting with physician
  • Assist in developing physical therapy plans for individual patients
  • Assist in transporting patient(s) to and from physical therapy room/area
  • Coordinate physical therapy with other departments
  • Perform administrative requirements such as completing necessary forms, reports, etc. and submitting to the administrator as required.
  • Participate in discharge planning, development and implementation of patient care plans, patient assessments, etc., as necessary
  • Involve the patient/family in planning objectives and goals for the patient
  • Document progress of residents by completing flow sheets daily to signify that the specific restorative nursing measure was done

Experience Requirements Prior experience in a skilled nursing facility preferred. Must be a minimum of twenty-three (23) years of age.

Must possess a valid and current Physical Therapist license in New Hampshire. Must have patience, tact, cheerful disposition and enthusiasm, as well as be willing to handle patients based on whatever maturity level, they are currently functioning. Must be in good general health and demonstrate emotional stability. Must be able to function independently, have flexibility, personal integrity, and the ability to work effectively with patients, personnel, and support agencies.


Plan organize, develop, and direct Physical Therapy services in accordance with applicable federal, state, and local standards, guidelines and regulations, and as may be directed by the administrator, to assure that the highest degree of quality patient care can be maintained at all times.
