The team including nurses, therapists, dietitians, social workers and physicians will design a health care program which addresses your specific medical and social needs.
Skilled / Sub-acute Care
The goal of skilled / sub-acute care is recovery after an injury, surgery or illness. This level of care generally requires intensive medical and rehabilitative intervention over a period of days or weeks to regain functional skills and return home.
Alzheimer’s Care
Hanover Hill is devoted to caring for residents with Alzheimer’s and related disorders. Our William F. Lee Alzheimer’s unit is staffed by professionals who are specially trained in caring for people with varying types and stages of dementia. An individualized program addresses medical, physical, intellectual and social needs. Residents receive compassionate care and assistance with activities of everyday living. Our goal is to enable residents to live fully and at the highest functional level possible in a safe, comfortable, supportive environment.
A respite care program is available to families who provide home care to a loved one with Alzheimer’s.
Long-term Care
Long-term care is designed for people with medically complex conditions which require daily nursing. Generally, residents who require intermediate care participate in a specialized program designed to enhance quality of life and promote independence.
